The 11th Annual Rampage Classic has teamed up with the Weber County Fair August 8 – 11, 2012. All games will be played at the Weber County Fair Grounds. Trophies will be given out to the first place winner of each bracket, medals for 2nd place finishers, T-shirts for all players and coaches gifts for every team. Every player will receive complimentary gate pass for each day of the Fair and parents will receive one day gate passes. There will be lots of venders, food, and activities for all ages, what a fun way to end the summer.

Date/Time: Tuesday, August 7, 2012, 6:00 to 9:00 pm.
Location: Weber County Fair Grounds, 1200 Fairgrounds Drive, Ogden, UT (Google map).
** The following documentation will be required at the time of check-in. ** Please put all player documentation in alphabetical order. ** Your team will NOT be approved to play until ALL documentation is provided. ** No players are allowed to double roster for this tournament.
UYSA Affiliated Teams
Official Team Roster: One (1) copy of Official UYSA/Affinity Roster (does not need to be signed by the registrar). This copy will be kept by the tournament committee.
Tournament Roster: Five (5) copies of UYSA/Affinity generated Tournament/Travel Roster. This roster MUST list every player and coach who will be playing with your team in the tournament, along with their photograph. All guest players must be included on this roster. Directions to create this roster can be found on the tournament website. One (1) copy will be kept by the tournament committee. One (1) copy will be used to track player discipline. Three (3) copies will be stamped for approval at check-in and you need to bring one copy to each game played.
Player Participation Forms: Every player (including guests) MUST have a player participation form (UYSA registration form). The form must contain signed consent for medical treatment. These forms will not be kept by the tournament committee.
