Online Athletic Department Release Forms

The procedure for completing the Athletic Department Release Forms has changed. All athletic realease forms will now be completed on line. Please click on the link below to access the online Athletic Department Release Forms.

Instructions for filling out the online forms.

Click on the form name and fill out the information requested.

Your student ID your PowerSchool Student ID (i.e. firstname-lastname).

• To sign the document click inside the signature box and hold your mouse down, this will allow you to create an “Electronic Signature”. If you make a mistake and need to start over, click on the refresh icon next to the signature box.
• Once you have filled out all of the information you will have the opportunity to print the document.
• You will receive a confirmation email once the document has been received by the athletic department.
• This electronic forms page must be completed and submitted for eligibility.

IHSAA Physical Forms

The IHSAA Physical form procedure is still the same as last year. The physical must be completed by a doctor on the official IHSAA physical form. Please make sure to sign all parent and student athlete signature lines. All IHSAA forms when completed must be turned into the Athletic Office. Please don’t return the forms to the coaches. We can’t accept the IHSAA physical forms.
