Physicals and Online Pre-Participation Paperwork Reminder

IHSAA Physicals

The IHSAA is using a new athletic physical form and all physicals must be recorded on the new form. Also, the physician must sign not stamped the form and include his physicians license number. The physical will be considered invalid without the physician’s signature and license number.

All completed physicals are to be turned into the Athletic Office.

ZCHS Online Pre-Participation Paperwork

The online Pre-Participation Paperwork must be completed by a parent or guardian.

Please note: the form will ask for a student ID#, the student ID is firstname-lastname (i.e. jane-doe).
