Physicals, Online Pre-Participation Forms, and Impact Test Reminder

The physical, pre-participation forms, and impact test must be completed. If all these items are not completed and turned into the AD Office (physicals), you will not be allowed to participate starting Monday until these items are completed. Please gets get these items completed as soon as possible as not to inundate the Athletic Department.

Also, there is only one more opportunity to take the Impact Test. See below for more details.

IHSAA Physicals

The IHSAA is using a new athletic physical form starting this year and all physicals must be recorded on the new form. Also, the physician must sign not stamped the form and include his physicians license number. The physical will be considered invalid without the physician’s signature and license number.

All completed physicals are to be turned into the Athletic Office.

ZCHS Online Pre-Participation Paperwork

The online Pre-Participation Paperwork must be completed by a parent or guardian.

Please note: the form will ask for a student ID#, the student ID is firstname-lastname (i.e. jane-doe).

Impact Test Reminder

All juniors, incoming freshmen, and any new athlete to the program must take the Impact Test. The Impact test is required to be completed before your first official practice (August 1^st) to be eligible to participate.

There is only one more opportunity to take the Impact Test prior to the start of the season.
