Physicals and Online Form Reminder

Thank you for all that already completed the physicals and online forms. The IHSAA physical form must be turned into the athletic office and the online forms must be completed prior to participation and tryouts on July 31^st

A hard copy of only pages 1-2 of the IHSAA physical form must be turned into the high school Athletic Office. The form must be signed (not stamped) by your doctor and have his license number. Please make sure to sign all spots for the parent/guardian and student athlete. Please click on the link below for a blank IHSAA Physical form.

Online forms can be accessed by clicking on the link below. You will have the choice of creating an account on rank one or just completing the online forms. Please note that when asked for student ID number you enter first name-last (i.e. jane-doe). You must have the hyphen and it is not a number.
